What counts as a judgment call?

A judgment call is anything you have not already given her a rule for: a complaint, a custom request, a price negotiation, a tone that reads upset. She would rather ask once than guess and get it wrong in your name.

Which inbox does this actually work from?

This works from the shared inbox that belongs to your whole space, the same one every channel feeds into. It is not tied to one individual email account, so nothing gets stuck because someone happens to be out for the day.

Can I see what she sent before it goes out?

Routine replies go out on the approval level you choose at setup. You can review every reply, review only the first few under a new rule, or let settled rules run on their own; the choice is yours.
